OFFICE APPOINTMENTS:

1. You can make an appointment with our office by calling 561-501-7445. Hit 1 for appointments, and please specify which location and doctor you prefer.
   
   
2. Complete our office form by visiting the FOR PATIENTS tab on our website or clicking on the red button above. Click on online forms. If you are using a laptop or cell phone, you can download the PDF Filler app or continue in the browser. Provide your name and email address, and hit the Get Started button. You will have to accept the electronic terms and proceed. Once completed, please make sure you hit finished so that we receive your copy. 
   
   
3. Bring in your picture ID (driver's license or passport)
   
   
4. Bring your insurance cards (health primary and secondary and auto insurance if related to MVA)
   
   
5. Bring your medical records from physicians who have treated you for the reason you are seeing our doctors. If you prefer to email them, JPuelles@brainandspinemds.com
   
   
6. Please bring your radiology studies (MRIs, CT Scans, X-rays, Myelograms). ** Please note that if you had your studies done at the following centers, we can access them online: Advanced Diagnostic, Beaches, Boca Raton Medical Center, DCA (Diagnostic Centers of America), Delray Medical Center, Independent Imaging, MRI Specialists, Palm Court, Stand UP, and University MRI.
   
   

TELEMEDICINE APPOINTMENT:

We are participating Medicare providers. Patients with HMO plans will need a referral from their primary care physician before the appointment.
